Originally there were 2 variations listed of the 5th issue blue Club Passport Card from the WinStar Casino in Thackerville, OK. Both cards had the reverse logo at the bottom left corner and the second card had an "01" over the mag stripe.

Turns out there are actually two variations of the first card with nothing over the mag stripe. One card has "Casinos" under "WinStar" in the logo on the back while the other card had "World Casino".
