Here's a newly reported Casino Privileges Select card from Montreal, Quebec Canada with a new reverse. There's still a single logo with a phone number under the logo but it's been moved from the bottom left to the bottom right on this card as you can see in the scans below. I'm still trying to find out if any other levels have the new back so let me know if you come across any.
