Appears to have been a customer loyalty card for the Jumer's Castle Lodge Hotel. The information I found seems to indicate that the casino printed the cards for the hotel and the hotel most likely gave the casino their customer list so they could print the names for them. That's why the ID numbers on this card appear very similar to the player numbers on the early slot cards. Neat find but not from the casino so not used for game play tracking, etc.
