Originally listed was a blue paper card from the Maxim in LV with the logo to the left but recently found there is a second variation is appears to be more grey but similar front design. Text on the reverse has a slightly different size font between the cards. On the blue card the bottom line in the reverse address ends under the "O" in "Casino" while it ends under the "A" in "Casino" on the grey card.
