Originally there was only one 5th issue slot card listed from Hyatt Regency Casino in Lake Tahoe but in reality all three levels (Players Advantage Club plus Gold and Platinum levels) exist in the fifth issue with the lighter silver line in the lower right logo. Earlier cards from the 4th issue had a dark grey line in that logo, otherwise the cards were identical.
