Pat didn't see this in the book. There are two of these cards for the San Juan Grand, Puerto Rico. One is for tables and one is for slots. If you look below the casino name on the right you can see the raised letter that says slots, my other card says tables. I got these 2 years ago, don't know if they are even still using them. I don't have traders.

Also the MVP on the Bally's Las Vegas slot card is now in gold, I do have some trader's of them.
