Be prepared to have a "story". Most of these type of places want you to fill-out their form and they hold the card for your visits. These cards do not get swiped (inserted) into the machines and basically become an ID card of your account.
Have a meal, ask for the application early on and think about filling it in. Perhaps leave with the application and card, which are usually stuck together.
