Gary - Jim is correct. The card was issued and used at both Hollywood Casinos (IL & MS) back in 1996. Pat/Steve have it listed in the book only under the casino in MS. I found mine in IL. They have it listed as EX RARE-100, with only 1 known card. Yours is now about the 4th or 5th card to be mentioned on this board in the past year, so the rarity/value may be going down in the next edition.
