Just found one last month at the Hollywood Casino in Aurora, IL. Asked at the club booth about it, and was told they were used for a "very short time" several years ago. The only way to get one now is to find one laying around. They are not even listed in my 1st or 2nd editions of the Wells/Lamb book, or in my 2000 copy of the Donley book. You just have to be in the right place at the right time.
