Mystic Lake is a HUGE place on the south edge of the Twin Cities. The location started with a little casino that was called Little Six, and that place is still open. But a mile way they have built a very big complex of gambling, hotel and convention center that is bigger than 95% of the casinos in the United States, Las Vegas included. The Guide is very confused about most of the older cards that came out of the Mystic Lake, and have not broken them down into types, just one big "Type 1" with many variations. If all the different cards could be examined this may be straightened out, as you can see they marked expiration dates on the cards so the people would get new ones every year. The black one shown has "03/98" printed on it (which makes me very surprised I found it on a machine top in May of 2001!) and the white one says "Expires 9/30/01." The gold Club Mystic 21 card is described in the book as a table card.
