Commerce Casino in LA, CA uses these cards on their electronic dealer-less poker tables. Players put money on cards like an ATM and winners are paid on the card to cash out at the cashiers window. Currently they are using them for a $40 Sit N Go. They aren't using the tables for "live games", maybe due to lack of player demand. The back side is blank. The cashier will print your name where it says "Lightning Poker". Unfortunately Commerce Casino doesn't have anymore of these cards, they're using blank white cards with a magnetic stripe on the back.

Hollywood Park and Hustler were using them, but they didn't go over too well with the players. I hear Las Vegas (Stations Casinos?) is suppose to try them out this month.
