This is from the Desert Inn. I scanned the back of the card because the front is black and hard to see. The funny thing is there is no hole punched in the card to attach the dangle. I remember standing at many Players Clubs and asking them not to punch a hole in the card. Some of them left a huge, ugly hole. Today most cards come pre-punched with a neat, small hole:
