Morning Larry!
The cards I have pictured at http://www.casinocardcollector.com/album_cat.php?cat_id=699 are probably what Pat refers to as the 5th issue. We made two batches of cards for them at the time (they order all of them together). On the first batch, the white cards did not have any arrows, so on the subsequent run, arrows were added. In fact, there are some cards out there that have thermally-applied arrows on them (after they issues some of the first white cards, they sent them back and we added arrows to the back with the same process that names are applied to the cards). So....techically there should be 3 versions of the white.
As for the other levels of play, as I recall there were no differences in the artwork/etc, and so only one version of each (not a 4th/5th issue).
Now, to add to the mystery.... I do know that California Plasticard did make a batch of the Yellow Elite version at one time (probably prior to our 4th version). I've only seen one example in the hands of a CPC sales rep. I cannot say if there's a difference in art/etc., as I had nothing to compare it to at the time. They may have also made a version of the the Red(orange)/Black version.
As for me - I'm still around (albeit a much lower profile). Enjoying the time with my girls with this time off (my contract with ACC finally expires in April). Working on a move back to the midwest where the wife's family is from - here's hoping for double-digit appreciation on the house!!! ;) I still pick up cards whenever I'm in Vegas, but really haven't had the opportunities to collect as much as when I still owned part of ACC. I check this board a few times each week to keep up on the happenings/etc. Good to still see everyone here!
