I picked up a few of these at the Cherokee Casino outside of Tulsa, OK. They are blank on the reverse (except for the mag strip).

I've seen a few from other Indian casinos in that area. Apparently Multimedia Games is a supplier of machines and they provide the casinos with these blank cards for use in those machines exclusively. The casinos print whatever they want on them.
