I have found 5 variations issued at Tuscany, Las Vegas. They are pictured in order.

The first issue was really a room key; the casino had not yet received the slot cards from the manufacturer, so they printed the name and account number on room keys.

The second issue had only one phone number on the last line of the reverse.

The current issue has two phone numbers.

After running out of cards, the casino issued these blue concentric boxes card as intrim slot cards. NOTE: there is no casino name on this card and the same card was used at 4-Queens recently.

The "Temporary Card" was also used while waiting for new cards.
