Barley's Casino & Brewing Company, Las Vegas is still open and issuing plastic cards, not sure about city, but I think they are in Henderson - check with Andy H.

There is a Eureka Casino, in Mesquite, NV issuing plastic cards- don't know of one in Las Vegas.
